Who are We?
We have GP Practice hands on experience and knew that we needed to improve our signposting of partient services.
By doing this we would significantly improve the demand upon our in-house appointments improving the number available for those patients who needed them the most.
Our staff were aware of the services they could signpost, but often forgot to do so when absorbed in the call with the patient.
We would also have new members of staff who had no experience or knowledge that would hold them back when answering the phones with any degree of confidence.

Faced with having to remind my staff everyday to signpost, I decided to design a simple to use, intuitive Toolkit specifically designed for use on the front line.
Our admin staff need to be able to direct the patient to the correct solution on every occasion and this Toolkit
empowers them to ask the right questions at the right time and then offer the right solution.
Use this Toolkit to demonstrate to the CQC that your staff can respond appropriately to all Patient appointment enquiries, and more importantly, can identify and respond to any RED FLAG telephone calls.

Each signposting / care navigation toolkit is designed to streamline the patient journey and improve the efficiency of your practice.
This Toolkit will support your admin team by providing the script they need to follow when answering a patients enquiry.
This means your clinical & and management team control the narrative to create a bespoke Toolkit that will enable your patients to be dealt with quickly, professionally, and directed to a team that are the most appropriate service for their issue.
This may be externally: Community Pharmacy, Physiotherapist, Sexual Health Centre, Minor Injuries Unit, A&E, Eye Care Clinic - the list is managed here to what is available in your area
Internally, the patient will be booked in with the right member of clinical staff and this may be: ANP, PP, GP, Nurse, HCA etc, again this would depend upon your team.
Through collaborative working with your PCN, there may be some shared services your Practice has access to as well.
